“BACKGROUND: The structural complexity of terminal

OBJECTIVE: To demystify TMC using clinical, imaging, ZD1839 molecular weight surgical, electrophysiological, and histopathological data and to propose a theory of embryogenesis.

METHODS: Our series consisted of 4 newborns and 6 older children. All had preoperative magnetic resonance imagings and surgical repair of the myelocystocele with electrophysiological monitoring.

RESULTS: TMC can be deconstructed into essential and nonessential features. Essential features are present in all TMCs and constitute the core malformation, comprising an elongated spinal cord extending extraspinally into a cerebrospinal fluid-filled cyst that is broadly adherent to the subcutaneous fat. The functional conus resides in the proximal cyst or within the intraspinal cord, and the caudal myelocystocele wall is nonfunctional fibroneural tissue. Nonessential features include variable measures of hydromyelia, caudal meningocele, and fat, present in only some patients. The core structure of TMC strikingly resembles a transitory stage of late secondary neurulation in chicks in which the cerebrospinal fluid-filled bleblike distal neural tube bulges dorsally to fuse with the

METHODS: We reviewed the levels of evidence across published clinical studies in 3 neurosurgical journals from 2009 to 2010. Randomized trials were evaluated by use of the Detsky Quality of Reporting Scale. Levels-of-evidence data for the same journals

in 1999 were obtained from the literature, and regression analysis was performed to identify predictive factors for higher-level evidence.

RESULTS: Of 660 eligible articles, 14 (2.1%) were Level I, 54 (8.2%) were Level II, 73 (11.1%) unless were Level III, 287 (43.5%) were Level IV, and 232 (35.2%) were Level V. The number of Level I studies decreased significantly between 1999 and 2010 (3.4% vs 2.1%, respectively; P = .01). Seven randomized clinical trials were identified, and 1 trial had significant methodological limitations (Mean Detsky Index = 16.3; SD = 1.8). Publications with larger sample size were significantly associated with higher levels of evidence (Levels I and II; odds ratio, 1.7; 95% confidence interval, 1.45-2.05; P = .001). the outcomes of carotid artery stenting (CAS) and open surgical repair (OR) for treatment of radiation-induced carotid stenosis (RIGS).

Methods: We retrospectively reviewed 60 patients treated for 73 RICSs from a group GDC-0973 ic50 of 5,824 patients who had carotid interventions between 1992 and 2009. Thirty-three patients (37 arteries) were treated with CAS and 27 patients (36 arteries) with OR. CAS was performed using embolic protection as part of a prospective institutional registry since 2003. OR patients with prior radical neck dissections had more wound complications (14% vs 5%) and CNIs (28% vs 9%) compared with those without neck Pifithrin �� dissections. Conclusion: Carotid artery stenting for radiation-induced stenosis has the advantages of no CNI or wound complications with similar early stroke rate compared with open carotid repair. Methods The procedural time for coiling was defined as the number of minutes between the first diagnostic angiographic run and the last angiographic run after embolization. Thus, induction of general anesthesia and catheterization of the first vessel were not included in the procedural time. A long procedural time was defined as the upper quartile of procedural times (70-158 min). Logistic regression analysis was performed for several variables.

Results The mean procedural

time was 57.3 min (median 52 min, range 15-158 min). More than half of the coiling procedures lasted between 30 and 60 min. The mean (SD) healing time in the SSB group (9.9 [0.77]) was shorter than that of the 4LB group (10.4 [0.80]) and the usual care group (18.3 [0.86]). Pain reduction was significant (P < .001) for the compression-treated groups only.
